The Cadence Health team is expanding and seeking a Patient Coordinator Supervisor to play a vital role in enhancing the patient experience, streamlining care delivery, and leading a team of Patient Coordinators. The Patient Coordinator Supervisor is responsible for monitoring patient cases for Cadence Nurse Practitioners, supervising a team of Patient Coordinators, and ensuring adherence to care plans in collaboration with Cadence NPs and the patient’s physician.
WHAT YOU’LL DO:
- Supervise and mentor a team of Patient Coordinators. Provide guidance, training, and performance feedback to ensure team effectiveness and adherence to protocols.
- Contact patients to review recommendations from Cadence Nurse Practitioners or the patient's physician, ensuring clear communication and adherence to treatment plans.
- Assist patients by providing reminders to complete lab orders.
- Conduct medication reconciliation with patients to ensure accuracy and adherence to prescribed treatments.
- Take responsibility for your tasks and your team's tasks while maintaining high standards of accuracy and professionalism.
- Ensure accurate and detailed documentation of patient interactions in compliance with HIPAA regulations and Cadence's clinical protocols. Oversee your team's adherence to documentation standards.
- Consistently meet deadlines and uphold reliability standards—for yourself and your team—to ensure patient care and service are never compromised.
- Independently manage your responsibilities and your team's responsibilities while upholding high performance and efficiency.
- Lead by example to ensure every healthcare interaction is exceptional—prioritizing patient well-being and aligning with the goals of our health system partners—and guide your team to do the same.
WHAT YOU’LL NEED:
- Active certification as a Medical Assistant from an accredited association (AAMA, NCMA, NHA, NCCT, AMT, NCCA) OR Active LPN/LVN license.
- 7+ years of clinical experience in acute or ambulatory care, with at least 2 years in a supervisory or leadership role.
- Must have a HIPAA-compliant home office setup with reliable internet, including stable Wi-Fi and strong upload/download speeds.
- Proven ability to work independently, manage time efficiently, fulfill job responsibilities with minimal supervision, and effectively supervise and manage a team.
- Maintain consistent attendance and adhere to work schedules to ensure timely and effective patient care, and ensure team adherence.
- Demonstrate strong written, verbal, and interpersonal communication skills, with the ability to effectively engage patients, healthcare providers, and your team.
