Position Summary
The Patient Health Information Specialist (HIS) is responsible for various health information management duties and tasks such as answering phones, scanning, faxing, uploading documents into the patient’s electronic medical records, process requests for medical records, processing mail, and maintaining the integrity of the medical record through assuring compliance with System, State, Federal and Regulatory requirements. This includes, but is not limited to, corrections, examining, and reporting.

Job Responsibilities

  • Receive and process requests for protected health information in accordance with Federal and State law, and internal Company policies.

  • Ensures that requests for medical information are fulfilled within the required time frame set by the HIPAA Privacy Rule, and Company policies.

  • Maintain confidentiality and security with all privileged information.

  • Prep and uploads relevant documents to the medical record.

  • Verifies patients using specified identifiers such as DOB, patient name, SS, patient address, and / or patient phone number before scanning individual papers and indexing documents into the right patient charts.

  • Learn the many types of documents and their placement within the chart.

  • Learn multiple computer software applications.

  • Receives and distributes incoming mail, processes outgoing mail, certified, and / or express mail.

  • Adheres to turn around time for Release of Information requests.

  • Receives requests for medical records via phone, fax, email, and mail and processes according to organization policy, State and Federal Regulations.

  • Reviews inbox for requests daily, prioritizes requests for processing and marks the tracking chart appropriately for the status of the request.

  • Enters new requests in record release tracking chart.

  • Answers the phone in a professional, kind, and courteous manner; offers help or transfers calls appropriately.

  • Understands the difference between a Power of Attorney, a Health Care Proxy form, and Living Will.

  • Understands Right to Access, Information Blocking, HIPAA compliant authorizations.

  • Collaborates with other departments as necessary.


Required Qualifications

  • High School Diploma or GED or equivalent work experience, college preferred.

  • Demonstrated experience and knowledge of the Health Information Management functions and departmental processes.

  • Demonstrated competency with electronic medical record systems and ability to learn new technology and software systems.

  • Effective written communication skills, including ability to write grammatically correct emails, messages, and other materials, as well as edit existing and create new department policies and procedures.

  • Ability to effectively communicate, both verbally and in writing, and to work with groups at various professional levels to accomplish goals and objectives while maintaining positive professional working relationships. This includes representing Caris in meetings, when communicating with patients/customers, outside legal representatives, and providers.

  • Conditions of Employment: Individuals must successfully complete pre-employment process, which includes criminal background check, drug screening, and reference check.


Preferred Qualifications

  • RHIA or RHIT certification or currently working towards these credentials.

  • Knowledge of State and Federal healthcare privacy and HIPAA regulations.


Physical Demands

  • Will work at a computer majority of the time.

  • Must possess the ability to sit and/or stand for extended periods of time.

  • Must possess ability to perform repetitive motion.

  • Ability to lift thirty pounds.


Training

  • All job specific, safety, and compliance training will be assigned based on the job functions associated with this employee.


Other

  • This position requires periodic travel and may work evenings, weekends and/or holidays.
